Cash or card: what should you use in Japan?

Cards work at many larger Japanese businesses, but some smaller shops, restaurants and ticket machines still need cash. Carry some yen and a separate backup card; an IC card can also be useful for transport and small purchases. Where did you still need cash on your trip?
